Custom TATTOO SUBMISSION FORM

To make a custom tattoo appointment please use this form and read info below.

Once your submission is reviewed and approved, I will confirm date and time, give a price estimate, and attach deposit info. Not all submissions will be approved.

Deposit - All tattoo appointments require a $100 deposit. Deposits are non-refundable and go towards the final price. Deposits should be made quickly to guarantee date and time.

Pricing - $200 per hour for larger scale work. Small/Medium scale work may be priced by the piece. $300 minimum.

Payment - Cash, Venmo, Cashapp

Design - Custom designs will be looked at in person on the day of the appointment. Small changes can be made if needed.

Reference - You’ll be able to send photos once we begin correspondence

Cancelation/Re-scheduling - A minimum of 24hour notice is required or you forfeit your deposit.

I Tend to stay booked out about 1-3 months. so please keep that in mind when booking.
